Waiver
I understand that some classes may involve tools and equipment with an element of risk of injury. I accept those risks for my children and myself. I release and/or indemnify Featherstone, its staff and teachers, against all claims by me as a result of our participation in classes at Featherstone. I agree to release Featherstone from any and all liability for the unintentional exposure or harm due to the Coronavirus (COVID-19).

Photo Release
I hereby allow Featherstone Center for the Arts to take photographs of me or my enrolled children to be used for Featherstone promotional and archival purposes only (Please contact the office at 508-693-1850 if you do not want your child to be photographed, or email [email protected]).

Emergency Authorization
I give permission for routine medical care for my child by the camp as well as permission to the medical personnel selected by the camp to order x-rays, routine tests, and treatment for my child. In the event that I cannot be reached in an emergency, I also hereby permit the physician selected by the camp staff to hospitalize, secure proper treatment for, and to order injection and/or anesthesia and/or surgery for my child as named above. This form may be photocopied for use off camp property.

Cancellation Policy
I have read the cancellation policy listed on the registration form (copied below):
Cancellation Policy/Refunds

• A refund, less a $75 processing fee, will be granted if you wish to withdraw your child from camp prior to June 1, 2025.
• A 50% refund will be issued if you cancel after June 1st but in advance of 2 weeks prior to your child's scheduled camp start date.
• Refunds will not be granted if you cancel within 2 weeks of your child's scheduled camp start date.
• Each weekly program requires a minimum number of campers. Full refunds are given only if Featherstone cancels the camp.
